I confirm that all information provided in this application is true, correct, and complete to the best of my knowledge. I understand that providing false or misleading information may affect the establishment of my SMSF.
I understand that Udo Super Services Pty Ltd (ABN 25 609 900 745) will act as my SMSF administrator and registered tax agent. I authorise Udo SMSF to interact with the ATO and ABR on my behalf to establish and administer my fund.
I acknowledge that I have read and agree to Udo SMSF's Terms of Engagement and Privacy Policy. I consent to Udo SMSF collecting, storing, and using my personal information for the purpose of establishing and administering my SMSF.
I understand that as a trustee of an SMSF, I have legal responsibilities under the Superannuation Industry (Supervision) Act 1993 (SIS Act). I commit to acting in accordance with those obligations, and understand that Udo SMSF will provide guidance but is not a financial adviser.
I authorise Udo SMSF to request a $611 ASIC fee reimbursement upon fund activation, as outlined in the engagement terms. This covers the ASIC registration fee for the corporate trustee company (where applicable).
